2938. A dwarf hamster has right neck swelling and left eye conjunctivitis

The son surfed the net and recommended Toa Payoh Vets for its proximity to lower Thomson Road.

An itchy swollen right neck area and itchy closed left eye. She was hyperactive and climbed up the cage.
She had been separated from sibling who used to bite her backside and she would just turn over to submit, but no chance of biting in past few days.

"The swelling could be an abscess or tumour," I said.
"Could the swelling be caused by piercing of a seed?" the father asked.
"Yes, if a sharp piece had penetrated into the lower neck. We need anaesthesia to examine. I will give 2 days of anti-inflammatory and if the swelling disappears or softens to an abscess that can be lanced. "
"If it is a tumor," the father said. "No operation as his immune system is poor and he may develop another tumour. So, no point operating to remove the tumour."  He had a long discussion with his son in their native language. The son was not keen on euthanasia.

I hope it is an abscess.

2937. What happens to urine marking 10 days after my removal of 2 abdominal testes in the poodle?

Toy poodle, Male, born 28 Jan 2016 in William Goh's kennel. Referred by William Goh to me for neuter on Sep 26, 2016 as the owner wanted an experienced vet in a practice that does not have different vets during consultations. The 8-month-old dog started urine marking for the past 10-14 days and the owner decided on neutering. The fabric sofa was urine-marked and the owner did not want the habit to persist.

Today, Oct 6, 2016, I sought feedback from owner
as to the post-op complications, if any, of bilateral abdominal cryptorchidism surgery which is rarely done.


Sep 26, 2016 neutered. I had given methone at 0.25mg/kg SC and tolfedine tablets as painkillers.

The methone painkiller is very effective in this case as:
1. Dog ate food the same night.
2. No self-inflicted skin bruises around operation area. Even with e-collar, some dogs
traumatize the op area using back paws to scratch to relieve pain.
3. Urine marking. Dog confined to a playpen with grate + pee tray. He was well behaved in elimination.
    Yesterday, he was let out and CCTV showed he urine marked the floor near the sofa. Neat vinegar + a fabric deodorizer solution with fragrance had been applied to the fabric of the soiled sofa area. Yet this dog still urine-marked.
"He still has residual testosterone hormone inside his blood," I had advised her earlier that neutering will not immediately halt urine marking.
"How long will the hormone be there?" the lady asked.
"Around 2 months," I estimated and advised confinement to the play pen area for a month.

4. "The knots of the stitches are still there," the owner said.
    "They are the thread-type, not the nylon type, and will dissolve in around 21-30 days," I said. "None of my neutered dog cases ever came back for stitch removal. I asked her to WhatsApp 3 pictures of the op area. She would do tomorrow.


 
In my over 40 years of practice, I have not encountered a dog with two abdominal testes inside. There are cases of one inside the abdomen. It is great that this dog had no post-op complications.

2934. A 5-month-old poodle vomits and has diarrhoea for 5 days

"Persistent vomiting and diarrhoea for 5 days," the lyoung ady owner was perplexed. "The stools are now becoming smaller and loose."

I palpated gas inside the anterior abdomen but there was no pain.
"Did the poodle swallow bones, toys, socks or other things," I asked as young dogs do put things into their mouth.

"No bones," the lady said. "But she did chew sponges from the sofa set."

She did not want blood test which would have indicated infection and state of the liver and lungs.
X-rays were approved and showed some impaction of the stomach and small intestines, with lots of gas bubbles. Pieces of sponges are possibly inside.






An IV drip with vitamins, antibiotics and pain-killers were given. The dog would be given a laxative to pass out the sponges.

2933. A Golden Retriever may have tick fever - Babesia gibonsi antibody test. A Presa had tick fever in 2014

In 2016, the supplier stocks Babesia gibsoni antibody test kits and Toa Payoh Vets have some. Yesterday, Sep 28, 2016, a young couple probably google "Imidocarb Singapore Vets" or some words similarly and phoned me.

His vet had no stock of the test kit. The 2.5-year-old Golden Retriever lives in an apartment and was unable to stand and had smelly diarrhea. He farted but could walk on his hind legs. He had a fever of 40.8 C and a lower RBC, Hb count of less than 5% in the low range. Platelets were very low at 36. The vet tested with Cani-Vet 4 which does not cover Babesia and so told the owners accordingly.
Anti-diarrhea drugs and electroltyes and prednisolone medication were prescribed.

So, the boyfriend surfed and phoned me. I had the test kit. The dog was negative to the test. If clinical signs are present (anaemia, seizure, fever) and this test is negative, it is interpreted as 2-4 weeks after infection. But the dog's gum are pink and though he has weakness in his back legs and diarrhea, this may or may not be a suspected case of tick fever.

Vet 1 did not want to give the imidocarb injection and so I did not want to interfere as the owner only wanted a test kit for Babesia and/or a PCR test.    

"Was a blood smear done by Vet 1?" I asked. The owner said a blood test was done. As the owner would be going back to Vet 1, I did not want to confused him further.

The owner wanted a PCR test which Vet 1 had advised.


PCR (polymerase chain reaction) is a technique that permits the analysis of samples containing only minute quantities of DNA or RNA. PCR is used to reproduce (amplify) selected sections of DNA or RNA for analysis.

At that time, there were no Babesia gibsoni antibody test kit. The blood smear was positive as the dog had clinical signs. Treatment was early and the dog is now normal.
