4557. Vet Case. Atopy. Poodle with right hip dislocation has a bad skin condition.

Follow-up on skin condition of the poodle with Ehmer Sling treatment 2 months ago. 

 

This 14-year-old poodle developed a bad skin inflammation, with wet fluid and "pus" appearing after bathing, for the past few weeks before treatment with Apoquel. The owner gave up going to Vet 1 as the dog would get a steroid injection often when the skin inflammation flares up. Vet 1 had advised food allergy trial but the owner did not accept this. 

Atopy is suspected. The dog was clipped bald by Toa Payoh Vets as the owner's groomer who went to the house did not clip the head and tail bald so that the dog would look pretty. Many groomers do not want to clip the dog bald from head to toe.  The owner remarked to me that she saw rashes and scales on top of the shaved head after clipping.

OUTCOME

With 1/8th of a tablet of Apoquel 16 for the past 7 days, the skin recovers to its normal appearance. But it is not a cure yet. The allergens and infections must be removed. Many owners think the skin problem has been resolved.

There is still a need to check for food allergy, environmental allergy (e.g. grass), skin mites and inhalation causes. Most owners do not have time to follow up with the vet. Some vets prescribe the same steroid injection since the owner has no time for many follow ups. In the end, the dog is not cured as it needs medication.

The owner stops treatment of the dog, as in this case. The dog wears an Elizabethan collar 24/7 while the owner bathes him with all types of shampoo, till the skin "leaks with pus" after bathing. That was the time, this owner consulted me, around 6 Jan 2023.

BUYING CHINESE NEW YEAR (CNY) PLANTS - 15 Jan 2023. 5pm

 22 Jan 2023 is Chinese New Year. Plants like orange limes and kumquats symbolising prosperity and wealth are purchased from nurseries. Video of Goodwood Florist which is a spacious nursery doing a brisk business in CNY plants. The usual small and big flowers in red or reddish colours as red is an auspicious colour for the CNY.

4555. A Private Sale is an excellent marketing strategy to reinforce loyalty

On 14 Jan 2023, I attended the 4th Private Sale for VIP customers by EU Holidays, expecting it to be the usual ordinary event with discounts if you book the tour on that day. 8 days more to Chinese New Year.  

How would the management organise this event to make it memorable and fun for loyal customers?

The event was very lively and crowded. The free Gelato ice-cream was popular. A Goddess of Fortune led by another young lady distributed oranges to all was a novelty as the God of Fortune is traditionally much older and male. A noisy tour auction to get cheaper tours e.g. the Netherlands Tulip Garden tour at $10.00/bid drew a crowded room with serious bidders.

An ang pow lucky draw on the office wall and a pretty mini suitcase filled with Chinese New Year goodies for those who sign up for the tour in addition to the travel discounts made the day for many who signed up.        

The experienced and attentive Mr Andy Yuan, Tour Manager was kind enough to pose for me to practise my indoor photography of him holding the mini suitcase as he was serving his customer. Thank you to Andy for being so sporting just to please another customer (me).

Here is the photo for Andy Yuan with best wishes and good luck for his 10-year career with EU Holidays. My wife and I had enjoyed two tours with him pre-Covid 19 and look forward to the Mar 2023 Finland tour with him.  

If I was the management, I will present him a Rolex watch for 10 years of long service. An experienced Tour Manager in the frontline brings in more business for the company and he is certainly worth a Rolex watch. If not, an Apple in appreciation of his hard work and sacrifices?

4553. Singapore Motorshow 2023 Suntec City 12 - 15 Jan 2023

 

The Hyundai Ioniq 5 First 100 is the first car to be assembled in Singapore in over four decades. 

It is the first EV to be built in Singapore – at Hyundai’s new HMGICS plant in Jurong. It looks like an ordinary Ioniq 5 but there are special “First 100” badges if you know where to look. An old poodle keeps licking her back belly area

5 Jan 2023. This poodle has a bad skin condition for many weeks. After bathing, the back areas under the belly became wet with "pus". The couple told me that there was no way the poodle could have had licked this area as she was on a big elizabethan collar 24/7. I advised the dog to be completely clipped bald and prescribed anti-fungal wash, antifungal tablets and antibiotics. An anti-allergic medication is given for 14 days. 13 Jan 2023:

4551. TRAVELS SINGAPORE. AMK Hub and AMK Central stalls on 12 Jan 2022 night.

TWO separate videos shown during my 12 Jan 2023 night visit to AMK Hub mall.

 TRAVELS SINGAPORE - 12 Jan 2023. Selling and buying Chinese New Year decorations at AMK Central stalls at night on 12 Jan 2023. A short video on the vendors. These stalls close after Chinese New Year which falls on 22 Jan 2023. Covid-19 curbs were lifted in Apr 2022. No need to wear face mask in public areas like this shopping area or malls.

TRAVELS SINGAPORE - 12 Jan 2023. AMK Hub Mall is crowded nightly as it is located next to the AMK subway MRT station and bus interchange. I went to Challenger to buy a USB cable to connect my Canon R5 to the laptop to download photos. I broke the camera card cover this morning when my tripod with camera was knocked down by my chair in the consultation room. Now, I have to wait for Canon to bring in the replacement card cover card from Japan. A video of some stalls in the busy mall.

VET CASE. Video 1/3, 2/3, 3/3.  What to do when you adopt a hamster with a leg fracture? 

12 Jan 2023 Consultation.

"I adopted this hamster via a Facebook advert which said that the owner had cats and could not keep this hamster!" the courier lady in her late 30s told me. "He was lame on the right hind leg, but I did not think it was serious."

"He has a lower leg bone fracture," I told her. "I could feel the sharp end of a broken bone protruding out of the skin!".
 

"It could just be hairs," my assistant commented based on observation, without examination.

"The white piece certainly looks like hairs," I replied. "However, I could feel it and it was sharp at the end."


My assistant agreed it was a fractured piece of femoral-tibial bone of the right hind leg. Physical examination is most important.

"You have two options for treatment," I said to the owner who has two Syrian hamsters.
"Option 1 is to amputate the infected painful foot below the knee. The hamster will live normally with 3 good legs.

"Option 2 is to treat the big infected swelling with medication - antibiotics and anti-inflammatory. The chances of success are very slim as the bone fragment had protruded out of the skin and infections had gone inside, leading to a big red swelling."

The owner said, "A dwarf hamster costs $20.00 only as compared to the surgery. But I have had adopted him and will help him." She decided to pay for the amputation surgery.    

This was a very kind-hearted lady. Surgery will be done 2 days later by Dr Daniel.

4549. A big smelly ear tumour grows itchier in a dwarf hamster. What to do?

26 JAN 2023:
The smelly left ear tumour was removed under anaesthetic by Dr Daniel Sing 2 weeks ago. The lady owner did not want the whole ear lobe plus tumour removed as advised by me. My reason was that the large tumour looked cancerous and could have had spread further. It will recur if cancerous. Removal of the ear lobe and tumour would save the owner money and stress. Since no biopsy is done pre-op and no histology of the tumour is done post-op (to save medical costs), I cannot be 100% sure. If cancerous, the ear tumour will recur within 8 weeks.

FINAL VIDEO. VET CASE. How to save money on vet expenses in large ear tumour in a dwarf hamster

VET CASE. A big smelly ear tumour. What to do?

The young lady had 2 dwarf hamsters. Matcha, male, 2 years old passed pale "white stools". Latte, female, 2 years old had a large inflamed red and smelly left ear canal tumour.

This article reports on Latte. On 4 Apr 2022, the boyfriend, a slim tall man in his 30s, came on behalf of the owner. "The ear canal tumour is large, over 5 mm across," I examined the tumour. "It is likely cancerous as it grows fast and also the hamster is old. Older hamsters develop tumours. She was 2 years of age. I advise a surgery to cut off the whole ear lobe with the embedded tumour."

"No removal of the ear lobe or tumour," the boyfriend said. "Let me try treating it with medication". The girlfriend had boarded this hamster with him. As she was busy working, he brought the hamster to the clinic while she made the appointment with me.

"Medication and washes will not be effective as the tumour is growing. It has a hole spilling out blood," I said. "Latte has had been scratching it with her paw as it is itchy."

The boyfriend insisted on medication against veterinary advice.

Two days later, I WhatsApp the girlfriend to follow up. "Her ear tumour is red and more swollen. There is bad smell from the ear," the girlfriend messaged back. "Will anaesthetic be risky if she undergoes surgery?"

"Much less risky as your hamster is active, not losing weight and is still eating well," I replied. "Let the vet know that you do not wish the ear lobe to be excised."

Dr Daniel Sing operated on 11 Jan 2023, leaving a large part of the ear lobe intact as that was what the lady wanted. She was most happy seeing the hamster's ear. Beauty to her and many hamster owners mean having two ears.

This Final Video shows that Latte recovers well from the anaesthesia and surgery. He is bright, alert and responsive today, 16 Jan 2023 compared to the period prior to surgery.  

Will the tumour recur? I do not know for sure as the tumour was not sent to the laboratory for histology to check whether it was cancerous or not, to save on veterinary expenses. From my observation of the fast growth and the old age of the hamster, I would rate this likely to be a cancerous ear canal tumour. It will recur, but we will know if it reappears within 2 months. 

 

HOW TO SAVE MONEY ON VET EXPENSES IN THIS CASE? 1. Early treatment when the ear tumour is smaller, less than 5 mm across. 2. Prompt treatment which is surgery. Medications are ineffective and permit the hamster to grow much bigger and redder. 3. Loss of one ear due to possible cancerous growth of the ear canal tumour. The loss of the ear lobe does not affect hearing. Only the beauty of the hamster. If the growth is cancerous, there will need to have another operation 2 months later.

4548. ENVIRONMENT - Arts message

The root cause of global warning must be addressed. This is the carbone emissions that continue to be pumped into the atmosphere at record rates using fossil fuels.bMajor emitters like China, the United States, India, Japan and Russia must transition successfully to a low-carbon economy.

Unprecedented floods and storms on one side and the deadly heatwaves and forest fires on the other, are alarm bells on two fronts. 

Global warning causes extreme heat and bone-dry vegetation. This results directly from excessive carbon emissions that trap atmospheric heat. 



Preventative measures include financial incentives, astute top-level signals and policy design and education campaign for a more sustainable economic growth pattern. Also to invest in substantial resource reallocation and technological advancement in energy production and consumption.
