INTRODUCTION

On 27 Jan 2022, a young lady consulted Dr Sing Kong Yuen. One of her two sibling 8-month-old red-eared sliders named TERRA had swollen eyes and no appetite for many days. Her internet research included a Youtube case from Toa Payoh Vets treatment of swollen eyes in sliders -   https://youtu.be/6AMqt7zDh3g

VIDEOS X 2

This video 1 showed Dr Sing and staff teaching the owner how to give oral multivitamins and antibiotics to TERRA. Despite Dr Sing's advices to hospitalise the slider for at least 3 days as he knows that the owner would not be able to give medication, the owner wanted to do-it-herself.

VIDEO 2. The owner's home video. She could not feed Terra. Any advice? The best advice was to let Toa Payoh Vets staff treat the young slider. We know from experiences that it is extremely difficult for the majority of first-time nursing owners in urbanised Singapore to get the slider to open his mouth to take medication. The owner hospitalised TERRA at Toa Payoh Vets.

The owner did not accept Dr Sing’s advice to hospitalise the anorexic and swollen eyed red-eared slider for 3 days so that the staff can give medication and eye drops to the sick TERRA.

VIDEO 3/4  RECUPERATION AT HOME

Video 8. At home.

NARRATE THE FOLLOWING:

7 Feb 2022. TERRA is now home. He still has infections and pain in his eye as you can see him PAWING his eye in the video.

He needs a long period of recuperation and nursing as he had not been eating for many days.

Terra has difficulty in grasping and swallowing pellets.

Video shows that the recuperating slider  has difficulty grasping the pellets. He has appetite but is unable to properly target the floating pellets and grasp them. "Why is it he cannot aim these days?" the owner WhatsApp Dr Sing. "Very time consuming for me to feed him by hand. We also fed antibiotics so many times but we wasted the medicine as he kept missing his mouth!"

Dr Sing advises more water depth, give smaller pieces of pellets and collect more medication from the clinic.

Video 9. 9 Feb 2022 at home. The slider is hungry but is unable to grasp the piece of salmon normally. The owner was very worried. Dr Sing advised her to be patient as the slider had not fully recovered from painful infections of the eyes and throat. He asked the owner to give much smaller portions of the salmon.

Within two weeks, the slider is able to grasp and swallow the pellets and salmon pieces normally.

VIDEO PART 4/4   UPGRADED HOUSING

Same video as the HOOK.

Narration below is given as the video is run.

Video 10. 1 Jun 2022. TERRA eats normally. He has no SWOLLEN EYES OR SHELL ROT.

The owner has upgraded the housing layout.

1. She bought a new filtration system.  See left side of of tank. It is a fish tank filter aquarium water filtration and oxygen pump from China, via online. It is silent in operation filtering the waste in the water. It increases oxygen in the tank water. Bacteria and fungi no longer accumulate inside the tank water causing eye infections and shell rot.

2. She bought a wider and bigger white basking platform so that the sliders have sufficient space to dry their shells under the ultraviolet light. (Show a different slide of the previous small basking platform with 2 sliders mounting each other).

3. The bigger glass tank with with a cover has dimensions  60 CM X 30 CM X 36 CM . Her cat cannot place his paws inside to "catch" the sliders and to dirty the tank water. Eye injury and infection may be caused by traumatic injury. For example, from the paws of the sibling slider hitting the eyes when both are crowded into the small basking platform. (show image of sliders one on top of the other.)

4. Sufficient depth of water now. The owner had added artificial leaves, ornaments and plants as before. The tank is bigger. The additions do not clutter up the tank as in the previous grey plastic container. There is space for the two sliders to swim freely.

5. No more Vitamin A causing swollen eyes now. Vitamin A is sourced from the vegetables and  pellets fed. Salmon and chicken meat provides protein for growth.

CONCLUSION

He is seen resting on artifical foliage at left of video. All harmony with the two sibling sliders at home now as they have much space to exercise and not injure each other by basking on a small basking platform as in April and May 2022

(show images of the grey plastic container compared to latest tank, the cat guarding the grey plastic container, the two sliders mounting each other on a small brown floating basking platform, not enough depth of water, Terra is sick image, Terra is well image).

4068. MALAR ABSCESS IN CAT. Gigantic facial abscess - cat fights. Two cases by Dr Sing Kong Yuen videoed.

CASE 1.  THE CAT WITH THE LARGE FACIAL ABSCESS – PARTS 1 AND 2.

Part 1/2 is this video is also shown on FACEBOOK on Mar 4, 2019. The care-giver contacted me at 10 am. He used to keep over 20 stray cats in a kennel place in Pasir Ris kennels some 20 years ago. Now he has this cat and another two cats in his factory shop in Ang Mo Kio.

This cat lived in the factory unit but roams freely within the factory according to him and had never been sick for over 10 years. "How long has the cat has a swollen face?" I asked him. "3-4 days," he said. That was an explosive ballooning of the cat's right facial area in just 3-4 days. The bad odour of rotten eggs wafted up when I incised the abscess. It indicated a flesh-eating gas-forming bacterial infection. What to do?

The cat was anaesthesized. Xylazine 0.01 ml + Ketamine 0.1 ml IM. Isoflurane gas and oxygen via gas mask topped-up the anaesthesia when needed. The swelling was lanced with a scalpel blade. Foul smelly thick brownish red pus flowed out of the cut. I used 3% hydrogen peroxide as an antiseptic after releasing the pus into the kidney dish.

Then I used copious amunts of normal saline to irrigate and clean up the inner part of the skin wound. Blood test showed very high numbers of white blood cells and neutrophils indicating a severe bacterial infection. Prompt treatment by the owner saved this cat's life as the infection spread rapidly within 3 days.

The cat did not have fever or become emaciated, showing that the owner did seek early veterinary intervention. The old cat woke up fast from anaesthesia as the dosage given was very low. The risk of dying on the operating table is very high in old pets, hence the use of isoflurane gas top up is the safest procedure rather than a higher dose of injectable sedatives. She went home with oral antibiotics and painkillers.

UPDATE:

As at Mar 9, 2019, no complaint from the care-giver. The cat had fully recovered and was eating as normal. He still wandered freely in the factory premises. Cat facial abscesses are due to cat fights. This cat was neutered. In general, it is best to neuter the male cat and keep the cat at the factory shop/home to prevent cat fights during wanderings.

This cat recovered fully after two weeks.

Pt 2/2 is at:

https://www.youtube.com/watch?v=0p6UnO0xX8Q

Video - Part 2 shows surgical treatment - lance and drain the abscess, dental scaling and extraction of the decayed lower right premolar tooth. Flea and deworming treatment were advised. WARNING. Part 2/2 contains surgical procedures on treatment of the malar abscess in this cat. Viewer discretion is advised.

P.S The upper right jaw has no more malar tooth when I examined the mouth during treatment. This infected upper 3rd premolar tooth in this jaw had decayed and dropped off before treatment. The bacterial infection of its roots would have spread upwards to the face over a short period of 3-4 days to form the gigantic facial abscess, also known as malar or carnassial tooth abscess.

CASE 2. Jan 29, 2016. A rare case of malar abscess in a cat. Dirty ears, Not groomed. Bad breath and drooling. 3 upper premolar and molar teeth extracted. The cat's facial wound healed well.
