4052. Final Video: A 4-year-old poodle dislocates her hip



Jan 9, 2018 case

Craniodorsal hip luxation is usually due to trauma. In this poodle, a helper stepped on the back, dislocating the left hip. Vet 1 tried closed reduction (without surgery) but the hip dislocated again. FHO (Femoral hip osteotomy) was performed by Toa Payoh Vets.


AFTER CLOSED REDUCTION by Vet 1, the hip dislocated again.






 
In this case, FEMORAL HEAD OSTEOTOMY (FHO) is being done.

X-rays confirmed hip dislocation has occurred.



 

The surgery is summarised as follows:
Blunt dissection of muscles
Expose femoral head more by using an assistant to abduct the joint
Bone cutter to cut off the femoral head
Create a false joint
Stitch up skin

4051. Femoral Head Osteotomy (FHO) in a Border Collie with follow up. FHO in a 4-year-old poodle

In August 2015, a Border Collie fell into a drain and dislocated her right hip.







I gave the owner 2 treatment options:
1. The Ehmer Sling which is not guaranteed to resolve the lameness. The cost is around $500.  If lameness is present after 2 weeks of the Ehmer Sling wrapped around the affected limb close to the body, there will be a need to perform the surgery called FHO.

2.  Femoral Head Osteotomy (FHO) surgery.  Costs from $1,500 to $2,500.

The Border Collie was limping on her right hind limb this morning. X-rays show the right femoral head has dislocated entirely out of the right hip joint. A Femoral Head Osteotomy surgery is advised.

"The chances of being back to normal without surgery are slim.," I said. "Without surgery, I can apply the Ehmer Sling. In this treatment, I use adhesive bandage to wrap around the affected leg and then sling it across the body. The dog may still be lame. The surgical option is likely to be lasting as most dogs do not limp after the surgery."


The owner wanted a treatment that is effective, without having to return to the vet again if the treatment is ineffective. 

"I prefer surgery."  The femoral head was cut off under general gaseous anaesthesia. The femoral head was excised.  No news from the owner till March 2017 (see below).

The owner came as the dog had swallowed a rubber toy.
I asked about the time it takes the border collie to recover normal gait after femoral head excision. "She took 3 months to recover and walk normally!"
