The old Jack Russell limps as she cannot put her right hind foot down - hip dislocation

On Jan 2, 2020, a 12-year-old, female Jack Russell became lame suddenly. The lady owner consulted Vet 1 who took X-rays and diagnosed right hip dislocation.

Vet 1 referred her to a practice saying: "The surgery would cost $4,000 - $5,000.". Her husband remarked: "Why pay $4,000? Just put your dog to sleep!"

On Jan 3, 2020, the owner came to Toa Payoh Vets. "Now that Vet 1's clinic is near my house," she told me. "I usually consult him. I did consult Dr Daniel Sing recently."

In 2020, there are nearly 100 vet clinics in Singapore, I understand perfectly that many owners will prefer proximity to save time and transport expenses.

My physical examination showed right hip dislocation. I had two better quality X-rays taken as Vet 1's X-rays were not sharp enough to show any femoral fractures clearly.

"I work part-time and so I don't earn much," the owner was furious about her husband's suggestion of euthanasia of her beloved dog. "I will pay all my salary, but I will never put my dog to sleep! I want her to be able to walk normally, without limping in pain!"

Dr Daniel Sing was her regular vet. He advised a non-surgical treatment using the Ehmer's Sling.

The owner consented. Dr Daniel sedated the dog and put an Ehmer's sling which holds the femoral head into the socket as much as possible.

He repeated the Ehmer's Sling again as X-rays showed there was still hip dislocation. The Ehmer's Sling positioned the femoral head inside the hip joint to allow healing. The sling would be kept on for around 2-4 weeks.

The Ehmer's Sling is a much less expensive treatment. It has around 50% chance of success. It is a safer approach since the 12-year-old dog may die under anaesthesia if she is operated. If it does not work, femoral head osteotomy (FHO), also known as femoral head excision can then be performed.

FHO involves cutting off the femoral head and removing it. A false joint will be created and the dog can walk and run without obvious signs of limping.

On Jan 9, 2020, the couple came to bring the dog home after 7 days of in-patient confinement. I keep my fingers crossed for the feisty Jack Russell and hope that she will walk and sprint normally in 2-4 weeks' time.

(Narrate as if speaking to a friend, varying your tone. Do not be monotonous)

PART 1
On Jan 2, 2020, a 12-year-old, female Jack Russell became lame suddenly. The lady owner consulted Vet 1 who took X-rays and confirmed right hip dislocation.

Vet 1 referred her to a practice saying: "The surgery would cost $4,000 - $5,000", Vet 1 told her. Her husband remarked: "Why pay $4,000? Just put your dog to sleep!"

On Jan 3, 2020, the owner came to Toa Payoh Vets. "Now that Vet 1's clinic is near my house," she told me. "I usually consult him. I do consult Dr Daniel Sing recently."

PART 2

My physical examination showed right hip dislocation.  (Edit and show physical examination video footage)

"I need to take better quality X-rays," I said. "To see whether there are also femoral fractures." Vet 1's X-rays are as follows:

(Poor contrast. The femoral head area is not sharp. Also narrate the text in the 2 X-rays).

Toa Payoh Vet's X rays show no fractures in the surrounding right hip area.
(Narrate the text and "arrow" in the X-rays)

Vet 1's diagnosis of right hip dislocation was confirmed.

"I work part-time and so I don't earn much," the owner was furious about her husband's remarks about euthanasia of her beloved dog. "I will never put my dog to sleep! I want her to be able to walk normally without limping in pain!"

Dr Daniel Sing was her regular vet. He advised a non-surgical treatment using the Ehmer Sling. Although it has around 50% chance of success, it is a safer approach since the dog may die under anaesthesia during surgery. If it does not work, femoral head excision surgery can then be performed. It is much less expensive.

The owner consented. Dr Daniel sedated the dog and put an Ehmer's sling which holds the femoral head into the socket as much as possible. (Narrate the X-ray text).

He repeated the Ehmer's Sling again. It ensures that the femoral head is kept back into the hip joint to allow healing. The sling would be kept on for around 2-4 weeks.

(Edit video shows the dog after treatment)

The common surgical treatment is femoral head osteotomy (FHO), also known as femoral head excision.

4047. Thick tartar in a 5-year-old X-bred dog. Dental scaling needed.

Jan 2, 2020. Thursday

"Can you prescribe some medication to dissolve the thick tartar in my dog's cheek teeth?" the man in glaring sunglasses asked me. He did not want his dog to undergo anaesthesia.

"The tartar is so thick and it must be cracked first. No drugs can dissolve it," I advised dental scaling. The dog was in a shelter as the owner could not accommodate him in his apartment. The dog also had itchy right thigh and rashes. So there were two medical conditions.

At 50% domitor and ketamine at 0.4 ml and 0.5 ml respectively IV, the dog was sufficiently sedated and dental scaling was done. Antisedan woke him up and he went home one hour later.

Treatment is by lancing and drainage. The slider recovers his appetite after treatment.

4045. Creative Writing - Writing for children: I'm hooked!. What is voice?

Writing Magazine. April 2019

What is your writer's voice?
Your use of grammar and sentence structure.
Your use of language.
Your tone.
And fundamentally, your personality and attitude.

How you see the world - and your own individual personality will shape your voice. It's absolutely crucial to be yourself when you are writing.  It is OK to have different voices for different types of fiction.

A strong voice and a stunning idea underpins a gripping plot and brilliant characters. Without voice and idea, your story is unlikely to take off.

An author with a strong voice is confident in knowing what they want to write and how they want to communicate it. If you read further books by the same author, it is like meeting up with an old friend. How do you gain confidence?

1. Don't try too hard. - more authentic voice to shine.
2. Be natural. Eg. If you write funny fiction, write things which make you smile and laugh.
3. Don't be afraid to be you. Your voice is unique.
4. Don't overedit. When writing pours out of you, it's more likely to be your natural voice. Overediting leads to you feeling insecure, unconfident

WHAT KIND OF VOICES ARE CHILDREN'S PUBLISHERS AND AGENTS LOOKING FOR?

1. Child-friendly - informal, conversational, exciting
2. Funny - humour (if done to your style) always works well with children
3. Playful language - dynamic and exciting.
4. Energetic tone - word choices that crackle and sparkle.

CASE STUDY: ROALD DAHL
James and the Giant Peach

Aunt Sponge was enormously fat and very short. She had small piggy eyes, a sunken mouth, and of  those white flab by faces that looked exactly as though it had been boiled. She was like a great white soggy overboiled cabbage.

1. The language is childlike - pggy, flabby
2. Humour - comparing James' aunt to a cabbage. Not a particularly nice cabbage.
3. Conversational tone.-  that looked exactly as though it had been boiled. She was like a great white soggy overboiled cabbage.
The 2nd sentence is like sharing secrets between friends, creating a bond between the author and reader.

4044. PHOTOS. Butterflies, Grass blue butterfly

1. Butterfly - Happy New Year greetings

Not a lesser grass blue butterfly-

Usually spotted hovering no more than a few centimetres off the ground, the Lesser Grass Blue (𝘡𝘪𝘻𝘪𝘯𝘢 𝘰𝘵𝘪𝘴) is one of Singapore’s more common butterfly species! 🦋

To tell apart this species from the other two ‘Grass Blues’ that have been observed in Singapore, check for the position and colouration of the submarginal markings on its wings.

While it may not be obvious here, it is also easy to tell apart the male and female butterflies! The male butterfly has a dull purplish-blue above while the female is brown and unmarked.

4033. Singapore Wildlife Stories. A large purplish moth, Hubner's wasp moth and a small golden moth seen in 2019, 2020, 2022

Dec 21, 2019
A Saturday. Sunny and bright.
I walked along the Seletar Simpang Kiri River.

Seletar Simpang Kiri River   21 Dec 2019

Blood-thirsty mosquitoes landed on my hand stealthily and on my long pants.

I had stopped wearing shorts to protect my legs from mosquitoes.
I was surprised to see this bigger moth and managed to take a sharper image.  It is at least 3 inches wide.

A bigger moth of around 6 cm wide.

For the past days, I see numerous smaller moths on the vacant grassland edge. One of them is shown below.  It is very difficult to focus on the eyes are the moth is thumb-sized or 2 cm wide.

Around 2 cm wide. Is this the younger version of the above?

MORNING STROLL along Lentor Crescent. A golden moth about 2.5 cm wing span fluttered and landed on the wall; near Bullion Park. I took several photos with my Canon DSLR 90D, 18-200 mm lens. Very difficult to focus.
