1217. Myanmar Stories: Helping a Myanmar orphanage run by nuns
I went to Khin Khin's office and met 4 Myanmar people of various social strata today. Two women are business women, one man is a Work Permit contractor and another woman works at the restaurant to support her child in a Singapore secondary school.
1. The business woman who wanted to give back to society
"I am not asking my rich friends to donate to my nuns' orphanage," she repeated to me a few times. " I want them to contribute to their own neighbour's needy and poor. But all they are interested is which European tour they will be going this December! Those who have succeeded should help the unfortunate. I bought a plot of land 20 feet x 60 feet for S$2,000 for the orphanage and now the head nun's and helpers live in the one-story building. If a donor can buy another piece, then the orphans can learn to farm and grow vegetation! I can teach them the values of hard work and responsibility."
I did not correct her use of "vegetation" at first and asked: "Is it swamp land? Many years ago, Florida people sold cheap land to international investors and it was swamp land!"
"What is swamp land?" she asked.
"Land filled with sea water and is unfit for building house or growing rice."
"No, no," she said. "The land is for rice growing. But it is not located near the main road and so the price is cheaper." Nowadays, Yangon's houses are selling for over S$500,000 for land size of 40 feet x 60 feet in hot areas not near to the downtown and millions of dollars in the downtown. Property prices there make Singapore properties look cheap. It is true but you will find it hard to believe.
"If it is $2,000 and the orphans can use it to grow vegetables which is the correct word, not vegetation, I am interested in donating this piece of land to the orphanage so that the orphans can learn some skills as well as getting a steady stream of income. Why not rear chickens for Kentucky Fried Chickens as these will earn them more money?"
"We are Buddhists. We don't believe in killing animals," she said.
"Grow some crops for the hotels' foreign guests," I said. "Higher sales value than vegetables."
"I propose organic farming," she said.
"Organic farming is not easy. The surrounding farmland is using insecticide and so, how can you claim organic crops are grown there when the next plot is full of insecticide spraying?"
"Ok, it will be vegetables."
"You must find a trustworthy person to ensure that the money is properly used for the orphanage, not into the pockets of the nuns or the staff or volunteers," I said. "Only then will there be donors to sustain the orphanage over the years. How about the realtor who buys properties for you?"
She took out her pre-paid card and dialled Yangon. Apparently it is cheaper this way. She spoke in Myanmarese which I could not understand. Then she said: "There is a seller who ask the orphanage to buy over the adjoining plot of land of 20 feet x 60 feet but the orphanage says there are no donors."
"How much is it?" I asked.
"S$4,000"
"You bought a similar plot for $2,000" I said.
"That was 3 years ago."
"Property had gone up 100% in 3 years or 33% interest per year. This is better than putting money in Singapore's fixed deposit getting 0.2% or some low figure!"
"Prices have gone up."
"Ask the realtor to negotiate," I advised. "She can be trusted and so I don't have any worries about being cheated."
1216. An old cat drools
In this case, the 13-year-old cat drools and stops eating.
No more drooling after removal of all back decayed loose teeth.

July 25, 2010 At Toa Payoh Vets
Blood in urine and difficulty peeing for past few weeks. Vet 1 palpated bladder stones and referred to another vet practice as Vet 1 does not perform more complicated surgeries and always refer to this practice. Very thin 4.6 kg, 39.1C. Urinary incontinence every 10 min 3-4X/day. Crepitus & gas inside bladder on palpation.

Urine test pH 6.5, bacteria +
Alternative to surgery. Owner was given acidifying tablets for 60 days, to feed C/D. No treats to be given.

Admitted for blood in the urine again. No fever. 4.4 kg, 38C
1.5 cm x 1.5 cm stones palpated. Does not want X-ray or blood test. Image of surgery taken to be posted.
Anaesthesia I used; Domitor 0.1 ml IV, Zoletil 100 0.1 ml IV. Isoflurane gas + O2 as required.
2 big urinary stones and >10 small ones in the bladder taken out. Hospitalised 6 days.

Dog had licked surgical wound. E-collar given when discharged 2 days of hospitalisation.
UPDATE AS AT NOV 26, 2013
Owing to financial constraints, surgery is done based on bladder crepitus on palpation which is quite accurate as regards presence of bladder stones inside the bladder.
No news or follow up from the owner. Phoned owner. left a message to call me back. Emailed owner.
No response. I presume the dog is OK for the past 2-3 years.
Large bladder stones cannot be dissolved by diet. No stone analysis was done due to financial restraints.
1214. Judgment Day for the Jack Russell - closed pyometra
A bright blue sky Sunday of Nov 24, 2013 was the Judgement Day for the Jack Russell.
Chances of survival are less than 50% as the X-rays showed a "dam about to burst soon". Blood test showed a raging bacterial infection with total white cell count over 46 (3 x normal).
What should the owner do?
Nov 25, 2013 was an unusually busy Sunday but I rushed this dog for X-rays myself and got the blood test done in the morning. X-rays confirmed large swollen uterine horns. Blood tests show a very high total white cell count and neutrophil numbers shot up too, indicating a sevre bacggerial infection..
"Schedule surgery at 4 pm," I said to Dr Daniel when he came for work at 12.30 pm. "The dog is very ill."
"She looks bright to me," Dr Daniel said.
"The dog had been given dextrose, glucose and amino acids IV and so she looks good. She could not lift up her head when she came this morning!"
The owner came and stitches were removed. Video of stiiches not completely closed.

Emergency spay after
a 24-hour I/V drip plus medication was done by Dr Daniel. You can see
the swollen uterus. I advised only isoflurane gas + Oxygen, no I/V
sedation as this dog was much lethargic. A speedy surgery ensures the
dog survives. Gas by mask was OK in this case. I assess that this dog
has a good chance of survival as the owner did not procrastinate
seeing the vet. However she was extremely lethargic and had slight
anemia. So Dr Daniel did not think she would be Ok post op. The dog
woke up just after the last skin stitch. This was a good sign.

CONCLUSION. As
at July 9, 2014, around 10 days post-operation, the dog was in
excellent health. She went home to a happy daughter of the owner who
had not wanted to spay her as he wanted her to have puppies.

1213. Video: A young guinea pig has a large leg abscess
Many Singapore guinea pig and hamster owners love their pets as family members in 2013 as compared to 20 years ago.. They would seek veterinary treatment early.
This "Be Kind To Pets" Veterinary Educational Video shows how I drain a ruptured leg abscess in a young guinea pig in Singapore. Sedation would have been given if the abscess had not ruptured so as to reduce stress and discomfort in the young guinea pig. However, there is always the anaesthetic risk of death due to sedation. In this case, sedation was considered unnecessary as the drainage of the pus would take only a few seconds by me.

1212. Video; A dwarf hamster has 3 gigantic chest tumours
Informed Consent is very important in veterinary anaesthesia and surgery. In this case study, the vet advised that there is a very high risk of death from blood loss due to the removal of the gigantic chest tumours, estimated to be around 20% of the 46-g bodyweight of the dwarf hamster. There is no other option but surgery for the owner as the tumours keep growing bigger and bigger.
Video shows history, physical examination and giving informed consent on the anaesthetic and surgical risks.
It is best to get chest tumours excised by your vet when they are very small as the anaesthetic and surgical outcome is usually excellent as there is less stress and loss of blood. The owner was informed that the tumours were inflamed and infected and the hamster would take 7 days of antibiotics prior to the risky surgery. He was told that this medication does NOT make the tumours disappear.
Find an experienced hamster vet as some dog and cat vets do not perform hamster surgeries.

Case TP 44770. Neepy the male hamster, 2 years
A "Be Kind To Pets" veterinary educational video teaching vet students how to do a physical examination and take history, receive informed consent to anaesthesia and surgery as well as histology efficicently by Dr Sing Kong Yuen, veterinarian at Toa Payoh Vets, Singapore .
Establish a good rapport with the new client by asking for relevant details and explaining the high anaesthetic and surgical risks since the tumours are more than one and large, necessitating longer operating time.
The hamster has been operated by Dr Daniel Sing excising two tumours at one go and is fully recovered. Histology of subcutaneous lump 1.5x1.0x0.5 cm shows a benigh tumour - benign spindle cell neoplasm
Zoletil 50 IM 2 drops sufficient analgesia to excise 2 tumours with scalpel blade
Injection IM 1.24 pm (A) First skin incision 1.26 pm (B) Skin stitched with 4/0 absorbable 1.33 pm (C). Time taken between end of stitching and first skin incision (C-B) = 7 minutes. No need isoflurane gas + O2 top up in this case.
The speedier the surgery, the better the chances of survival for dwarf hamsters
