Vet Case Study: Feb 2025. RTA in a Corgi in Myanmar.

RTA (Road Traffic Accidents) in pet dogs are common in some peak-hour busy roads in Myanmar.
Many pet dog owners live in shophouses fronting the main road. The pet dog runs out from
an open door and dashes across the busy road. Many get knocked down by vehicles, suffering traumatic injuries like leg fractures and death.


This video discusses the vet case of a non-displaced fracture in the distal 1/3 femur of the Corgi.
The femur is cast and splinted. This treatment is cheaper than open surgery using pins and screws. However, the owner must ensure that the dog does not bite off the cast and will seek reviews 1-2 weekly especially if the cast is loose or the skin is infected, inflamed and swollen.

In this case, the dog bit off the cast. The owner did not want re-casting.

4485. Terrapin eating disorder

 

Toa Payoh Vets.

27 Jan 2022. 15 g. Juvenile bought.
4   Feb 2022. 17g. Anorexia. Swollen eyes.

13 Jan 2025. 215g. Poor appetite. Syringe feeding.
21 Jan 2025. 222g. Review.
5 Mar 2025. 204g. Anorexia. (Eyes closed, lethargy, not swimming, floats).

5 Mar 2024. Lunch meeting. Wt. 204g for 3-year-old male RES. Emaciated and stunted.


Her cat with chronic skin disease disappeared for 6 months. Then full coat when found.
Go to public pond? Will start eating and put on weight. 


Stressful of daily spoon feeding. Travelling job. Eyes now closed. Anorexic. Floats. Don't swim. Lethargic. .


Not likely to survive.

Syringe feeding

1. Food from nose --- go to lungs.
2. Eyes closed -
3. Not swimming.

4. Passed stools on towel when syringed fed for past months. But very stressful and time-consuming.

Critical care

Antibioitic eye drops

Multi-vitamin plus Calcium

Antibiotics

Heat water tank to 30 deg C. Not functioning.

WATER TANK.

1. 21 litres of water.

2. Thermometer at 30 deg C, but not functioning over past 24 hours. Return to vendor.

3. Added 2.5 ml "blue chlorine" to neutralise the chlorine in the tap water.

Methylene blue for anti-chlorine given for past months!



4484. Urolithiasis

 A 2 years old male spaniel dog has no urination for 4 days.
Yesterday, they visited a clinic to check the dog's condition. Today that clinic referred the dog to our clinic as 2nd option.
I take X-ray to see any blockage inside the urethra and found one urolith near os penis.

Surgery is needed to take out the urolith but the dog is  very lethargic and likely post renal azotemia.
Dehydration is ~5%.
So, I didn't decide to do surgery immediately.
I tried to push back that urolith to the bladder by flushing. But couldn't. So, I decided to do cystocenthesis for now and giving drip for 24hr and will do surgery tomorrow.

 

 

 You should have taken a V/D x-ray as that is important.

 How about blood test results?

 It is important you do close up on the prepuce and urethral bend, to show more details of the stone. Not the whole abdomen

4482. Vet Case Study. A needle with thread is found in the cat's stomach.

 Vet Case Study. A cat swallows a sewing needle with thread. What to do?

On 9th January, 2025, this 7-year-old female domestic shorthair cat swallowed a needle and thread.The worried owner consulted Dr Thet Hmue Kyaw in Yangon. Dr Thet’s X-ray showed the long sewing needle was lodged across the cat’s stomach.



HOW TO RETRIEVE THE SEWING NEEDLE FROM THE CAT'S STOMACH. 1. Endoscopy. 2. Artery Forceps with full surgical incision. 3. Endoscope with full surgical incision - this vet case study.



ENDOSCOPY AND FULL SURGICAL INCISION METHOD --- this vet case study.





A full surgical incision is needed. The abdomen is incised. The stomach is then incised. The vet inserted the end of the endoscope into the stomach and used its forceps to retrieve the needle. Near the incision, the needle had to be positioned such that one end of the needle could be taken out by hand. The cat recovered fully and went home.

ENDOSCOPY

Endoscopy is the insertion of a long, thin tube directly into the body to observe an internal organ — such as the gastrointestinal tract — in detail.

 

ENDOSCOPE.
An endoscope is a medical device with a light attached. It is used to look inside a body cavity or organ. The scope is inserted through a natural opening, such as the mouth during a bronchoscopy or gastroscopy.

 

ENDOSCOPY

Dr Thet made a small incision in the Greater Curvature of the stomach. He inserted the endoscope and manipulated its forceps so as to grasp the sewing needle towards the incision site.The forceps could not take the needle out of the stomach as it was long. Dr Thet positioned the needle next to the incision site and took it out by hand.

Can endoscopy be used to take a sewing needle out of a cat's stomach?

 

Yes, endoscopy can sometimes be used to remove a sewing needle from a cat's stomach — but it depends on a few key factors:

When Endoscopy Is a Good Option:

  • The needle is still in the stomach (hasn't passed into the intestines).

  • It hasn’t caused a perforation (puncture) or major bleeding.

  • The vet has the proper equipment and experience.

  • The cat is stable enough for anesthesia (endoscopy is minimally invasive, but still requires sedation).

When Surgery Might Be Necessary Instead:

  • If the needle has moved beyond the stomach into the intestines.

  • If it has pierced the stomach lining or another organ.

  • If the endoscope can’t grasp or retrieve the needle safely.

  • If the cat shows signs of internal injury (fever, lethargy, vomiting, pain).

How It Works:

Endoscopy involves inserting a thin, flexible camera (endoscope) through the mouth and into the stomach. Tools can be passed through the endoscope to retrieve objects like a needle. It avoids a full surgical incision, so recovery is quicker and less painful — when it's feasible.

 If you think your cat swallowed a needle (or you're dealing with this now), it’s very important to act quickly. Time matters — the longer it stays in, the higher the risk of complications. An X-ray or ultrasound is usually done first to locate the needle.

This is 

 yesterday morning. 

I used Ketamine and Xylazine IV as induction,
Then placed ET tube and maintained by Isoflurane.
I made a small incision on the body of stomach and take out the needle with the help of endoscope.
Then stitched up with polydioxanone monofilament 3-0.

I just mentioned the equipment I used in the surgery.
 
Needle couldn't be palpated over stomach. I don't want to make large incision at the stomach if possible. So, I made only small incision and tried with endoscope to find and grab the needle and It was ok.
I will send the photo tmr.

 

Fasting after surgery for 48hrs. No vomiting, good Bright alert and responsive the next day.
The cat was discharged from hospital due to cost matter.


 I did gastrotomy and take out the needle under GA.
The cat is currently fasting. But it's in stable condition.

Yesterday, I was busy with operations and OPD. I forgot to take photo. 

It was a needle with about 6 inches thread.

I had to incise the stomach but didn't want to make a large incision. So, made a small incision at the stomach and placed endoscope's wire inside and then grab the needle with forceps from the tip of endoscope. Actually, needle couldn't be completely taken out of the stomach. I grab it just to the incision site, then take out by hand.

It has to be connected with the top of black wire where a tiny camera exists. I can't see the camera inside.
It has to be connected with black tube wire and forceps snare for full installation.
I don't remember clearly how to install it. If you want, I can ask my colleague the next day.


 

4481. Cat fractured distal femur

 The cat is too active and aggressive
and owner couldn't manage to restrict movement. So, the cast was taken out by self at home.
After that, growth plate is totally displaced from the bone.

This X ray was taken yesterday.
You can see totally displaced growth plate and also there are high density foreign bodies inside the stomach. That might be the cast bandages.

4480. Myanmar. Immune disease Alsatian

 Immune mediated haemolytic anemia suspect in over 2 years old male GSD.

 

 I will send blood tests and detail history and clinical signs next day.

 

 Over 2 years old intact male GSD presented with lethergy, reduced appetite and weight loss, jaundice.
Dog had tick infestation last 5months and owner gave Simparica tablet that's for external parasites.
Then, gradually reduced appetite and weight.

At the time I do physical examination, the dog is too weak, jaundice and also mild dehydration.
I assumed that jaundice might be due to hemolysis rather than liver issue. So, I check CBC, PCV and liver panel.
And Anemia+ as the result of CBC.
Reticulocyte+ in CBC, so, regeneration is noted.

Jaundice+ Anemia
So, the underlying cause of anemia in this case might be RBC breakdown rather than reduced production and ongoing blood loss.
So, as differential diagnosis, the underlying problem might be..

- Sepsis
- Blood parasites
- neoplasia
- Autoimmune


The dog hasn't any sign of sepsis.
Negative for Snap4Dx test
Didn't see any tumor mass in X-ray.

So, that over 2years old GSD might have immune mediated haemolytic anemia in my opinion.

 

 PCV 16% today.
Still searching for donor. Haven't got  yet.
The dog is on saline drip, vitamin and iron supplement, and prednisolone 1mg/kg S/C BD.

 

4479. Cat fell off grooming table - Respiratory distress

 A 3 years old Persian cat was presented by respiratory distress after an accident (falling down from grooming table) at Spa. The owner went to the nearest clinic. After X-rays, the case was referred to us.

After taking complete physical examinations and history taking, I gave the cat lasix, dexamethasone, VitK3 injection, Antibiotics (Flumoxacillin and Metronidazole), vitamins and oxygen supplementation. 


For the following days, I gave same antibiotics, pain killer (Meloxicam) and B vitamins. And also nebulization.

 

 

23 Feb 2025

 Today, I check X-ray again at our clinic. The cat is in stable condition except mild rapid breathing.

 

 There is no fever at all. I just told the owner to discharge from hospital with oral tabs. Will review in next 5days, I will take 30sec vedio of the cat on that day.

 

4478. Ask and you shall be given

 On 5 March 2025, I had lunch with Abigail. She is in her early 50s and had been to Africa, Europe, the Americas and Asia to market her company’s products.

On 5 March 2025, Abigail who brought along her stunted and anorexic red-eared slider to consult me invited me for lunch. She was going to leave it in a park's pond with other sliders as she would be travelling again. 

 

She had been to Africa, Europe, the Americas and Asia to market her company’s products for over the past 20 years.

 

She is in her late 40s, medium built and 5 feet 5 inches tall. But she is full of beans.

"As a single lady frequent traveller, did you encounter dangerous situations?" I asked.

"One time in Greece, I had to catch the last train from the airport to my hostel," she said. "The airport information girl gave me a map and asked me to catch the last train to a station. There I had to catch another train to reach my hostel!"

"I dare not take a taxi as it is not safe for me" she said. "So I had to carry my heavy luggages and things myself." I went up the escalator. I asked God for help. (Ask, and you shall be given. Seek, and you shall find).  A man was present and I was directed to him. "I am taking the same train," he said. You don't need to take a second train to your destination!"

"How do I describe him? He was not the tall, dark and handsome - such types would make me worry about my safety. How do I describe him? It is hard."

"He is the a mousy man," I said. "Someone quiet and shy such that people do not notice him!"

He said he was working in this place and was going to the same train home. I asked him for help with my luggages. He picked them up and walked fast ahead."

The airport train station was packed with tourists. He put down my luggages and climbed up to his cabin. I thought he would carry them for me up to the train, but he didn't because I was too proud to ask for help. (ASK, and you will be given).

 

"My train arrived at the Station. This time, I asked him to help me with the luggages to my hostel. I don't stay in hotels as that would be more expensive.

"At the hostel, I asked him to exchange phone numbers, but he declined and left!

 Aedes aegypti is the main vector of Dengue fever, while Aedes albopictus is the secondary vector

 It is the Aedes albopictus (Asian tiger mosquito or forest mosquito). It is the secondary vector of denque fever.
