3393. INTERN. A 7-year-old red-eared slider cannot lay more eggs -oxytocin and calcium 2 hours

"I don't know whether you believe this or not?" the owner said. "After my son rescued this red-eared slider 2 years ago, my business started to flourish with big deals kept coming in. Before that, I had very small income!"

"Doing kind deeds do get rewards sometimes," I believe that there is divine intervention for some people with faith.

3/8 eggs laid naturally on the soil at the Vet clinic. X-rays showed 5 eggs. Oxytocin and theracalcium given. Laid 5 eggs.






Feb 16, 2019.
Laid 1 egg.  Feb 17, 2019. Laid 1 egg. No digging signs.  No X-rays

Apr 20, 2019.

In Apr 9, 2019, the slider was restless and digged occasionally. 
Came for injection as the slider had furiously digged the soil and flooring for the past 2 days.  Oxytocin and theracalcium given as before  X-ray on Apr 9, 2019 showed 5 eggs but the owner did not want the injections.

His wife treated this slider like a family member. She would worry when the slider did not lay eggs. "My slider would voice 'pee-par, pee-par, pee-par' and stop eating when she wanted to lay eggs. I have a bathtub of sand but she was not interested.  So I brought her to consult you!"

"Maybe she was not due or the uterine contractions were not strong."

I gave the slider the oxytocin and calcium injections after the wife debated whether to let the slider give birth naturally or not. See video of consultation.

"Don't hang around watching her in the tank of water," I advised the couple to go to the coffeeshop for a drink for one hour. "The slider needed silence and peace to lay eggs!"

"Any eggs laid?" the couple returned one hour later.

"There is none, but you can bring her home to wait".  
The wife wrapped the slider in a large white towel and put her in the back seat of the car with her.

The air condition was switched on.  After 10 minutes, the husband came to me. "One egg had been laid," he was very happy. I rushed out to view. After another 10 minutes, another egg was laid.





After 2 hours after injections, the slider laid the 5 eggs inside the car's back seat.

3 pm.
Laid 3 egg on soil outside the vet clinic before X-ray could be done. She quickly covered the eggs.



The X-ray showed 5 eggs inside.











3.45pm injection of oxytocin and calcium.
No eggs after 1 hour. Brought slider back to the back seat of the car.

Soon, one egg was laid. By 5.45pm, all 5 eggs laid in the air-conditioned back seat of the car.

3391. INTERN. FIV in a strayed cat. What to do?

He was born free.   Away from the confines of an apartment or control of the owner.
He roams outside his neighbourhood and defends his own turf successfully. He may be small in size, but his heart is big. Claws out when he meets other male feline intruders. No serious bite or scratch wound in the past years.  He lives free. Roams everywhere. See the little bird. Pounce on him! Lots of fun when one is free!










But freedom comes at a price sometimes.

Stray cats are a rare species in the hawker centres and food courts (images) as the Singapore Government had culled them in the past years. Now there is a TNR program and Singaporeans do meet the occasional stray cat.

This family of 2 parents and two pre-teen daughters feed this stray cat in the void deck (image of HDB void deck).  He has freedom to roam. "He will fight off any intruders into his turf," the younger daughter told me.




On New Year's Day, he was brought in with serious coughing. Non-stop coughing. Why?

FOLLOW UP ON JAN 10, 2018
The cat is active, has appetite and not coughing. Eating wet KD food. Continue antibiotics. Does not want to stay in the house.

3390. INTERN. 4 cases of in-patient red-eared sliders at Toa Payoh Vets

A RJC Intern brought a very small note-book to scribble notes on case studies. "Not practical," I told her as notes needed to be written fast. A4-sized papers were given to her!






3389. INTERN An obese jaundiced cat. Is there any hope of recovery?

An example of how to produce a Be Kind To Pets veterinary educational video. A movie structure is used. It starts with a "hook".

1.  HOOK
Your beginning must be strong visually as well as emotionally. Start your video with either

          a) A visual HOOK (atmosphere or action), e.g a problem has occurred.
          b) Puzzling event e.g. yellow 3rd eyelids, ears and gums in this case study
          c) Verbal argument
A short video clip of 1 minute is preferred. Images can be used if there is no video footage.

In this video, we will use footage of one video on Day 6 of treatment.

Jaundice - evident by yellow 3rd eyelids, ears and gums.

Yellow 3rd eyelids and ears in a cat are not normal. The cat has yellowish eyes, gums and belly skin. Why?



Look at one of these two normal cats. The ears are white. What causes jaundice in this cat?

3.  SETTING
This cat lives in Singapore, a city-state of around 6 million residents. Over 80% of the residents live in apartments. Sengkang is a new neighbourhood with many apartments recently built.
(Show Sengkang West pictures are you narrate). There are other cats living in the house.

4.  DEFINITION OF JAUNDICE


5.  DIAGNOSIS
History, Physical Examination, Laboratory test (blood test, urine test, X-rays, ultrasound...)

Show BLOOD TESTS Done using a table.
The 3rd blood test showed liver enzymes have decreased (show figures). This means that the cat is recovering from liver disorder.  However, the cat has kidney failure as evident by the high creatinine and urea levels (show figures). He is on daily dialysis with 200 ml of fluid under the skin till the kidneys recover.

Blood tests of total white cell count, neutrophils, lymphocytes, platelets (show figures) show no abnormality. This indicates that the cat has no blood-borne bacterial or viral infections. The cause of
jaundice is likely to be hepatic lipoidosis

6.  TREATMENT
The cat was given anti-vomiting for the past 3 days and hand fed Hills' A/D and K/D. The cat has improved slightly based on absence of bilirubin in a recent urine test.

A 3rd blood test showed lower liver enzyme indicating that the cat's liver is recovering from treatment including Ornipural injections. However, the creatinine levels are high indicating kidney failure. The cat is on daily fluid therapy via subcutaneous route. No more vomiting as at Jan 1 as the cat had anti-vomiting injections for the past 5 days.   

7.  OUTCOME
as at Jan 2, 2019
Out of 4 cats, this cat eats much more at a young age. She is the only one to become obese. It is likely that she is suffering from hepatic lipidosis  (define).
As at Jan 3, 2019, the ears and eyelids are less yellow. The cat is on daily dialysis with fluid.
