3381. A 10-year-old Shih Tzu had right eye tearing.

After 10 years, I saw this dog today. The dog had a bladder stone removed at the age of 2 years and so far, has no recurrence. The fluorescein eye stain show green spot in the centre of the right eye cornea. So there was a corneal ulcer due to the other dog's pawing him.

E-collar and painkillers.




3380. A 14-year-old spayed female Cavalier King Charles has urinary incontinence - Stilboestrol 1 mg tablets

The dog was lethargic, head down and could not stand long. "My old maid went home. The new one is with me for 3 days only caring for the dog. The dog had diarrhoea 3 times yesterday, after a short walk. It was going to rain dogs and cats," the lady owner said that the dog did not eat any grass or pebbles.

"Some few years ago, she had ingested 2 pebbles causing obstruction and you operated," she recalled. However, I did not confine her and her guts came out. So she had another operation. Soon after, another operation for the stitch breaking down. In total, she stayed 6 weeks as an inpatient!  I had to remodel the garden as she is a scavenger dog!"

In October 2, 2018, Dr Daniel prescribed stilboestrol 1 mg tablets to be given for 5 days and then 1 mg weekly as required for maintenance.   The dog had urinary incontinence for 10 moths. 

"It was great as the dog had no urinary incontinence for a few weeks," she reported as I complained that this dog has a strong urine smell in her back area indicating urinary incontinence now. "I gave around 7 tablets and lost the rest. I did not bother. So urinary incontinence returned for the past weeks"

Pus was present in the right ear too. The dog had dry eyes and moved the head away when I palpated the right ear. Ear irrigation was necessary.

"Vomiting and diarrhoea may be due to kidney disease," I requested a blood test. The owner gave consent. The dog had good heart, lungs and no abdominal pain or swelling. She just could not stand up and I suspected a bacterial infection in the blood stream.

2 days of inpatient are need to treat this dog. It is important that the old dog be given proper medication to prevent urinary tract infection and incontinence or ear infections as the old dog has a poor immune system. She gets infected easily unlike a young dog.

3378. Paw biting and ear scratching poodle. Apoquel trial on Dec 20, 2018

"You did not have much time to treat the poodle for paw biting," I told the retiree owner. "All 4 paws have infected skin with flaky scales!  Your dog also has roughened ear pinnae skin which is not a normal situation."

"I thought it is normal for poodles to bite their paws daily," the man said.
"It is not normal."

His poodle attempted to escape from the reception room by pawing the glass door. He figured out that the vet surgery is not good for him!



















Treatment was ear irrigation and hair plucking under Domitor and Ketamine sedation IV,
dental scaling and paw clipping.

An initial dose of baytril and pred SC was given to last the first 2 weeks.
Then, Apoquel 3.6 mg,  1 tablet twice per day for 14 days and maintenance dose of 1 tablet daily for 14 days.

3376. A pet mouse is very itchy in the face and above the neck. Why?

4 days after removal of 9 warts by Dr Daniel at Toa Payoh Vets, the owner sent the pet mouse for boarding, asking the operator to apply the medication (oral, cream, wash, powder). The mouse felt very itchy and as you can see, the skin above the neck became bald. The skin below the left ear was rubbed bald. So was the right side of the nose, making it red.
















It is best to ask your vet to hospitalise the pet mouse for at least 7 days as nursing can be properly done.




FOLLOW UP ON JAN 10, 2019
Owner said the inflammation is much less and was satisfied. Itchiness is lesser in intensity.  He wiped the wound with clean plain water daily using cotton bud. The mouse does not exercise nor enter the small houses.

3375. Miniature Schnauzer skin disease: Control of Atopic Dermatitis using Apoquel (oclacitinib tablet)

Today, a dog owner was very satisfied with the disappearance of pruritus and skin flakes in her Miniature Schnauzer that had atopic dermatitis for the past 4 years.
"His medical bills cost more than mine," the woman in her 40s did church work in Myanmar, helping out with the drug addict problems.
  
"Look at the positive side," I replied. "You are healthy. Do you want to incur higher medical bills than your dog!"

The dog was on apoquel 3.6 mg 2x/day for the last 14 days. The vet did not give maintenance dose of 1 tablet 1x/day. He was on antibiotics, ninazol and a pred inj in the first 14 days.

Food allergy tests showed the dog was allergic to duck, turkey, pork and fish. So the owner now feeds chicken, rice, carrots and brocolli. The amount of stool is less compared to the dry dog food.




Apoquel is a synthetic Janus Kinese (JAK) inhibitor.
Dosage is 0.4 to 0.6 mg/kg, oral, twice daily up to 14 days and once daily for maintenance therapy, with or without food. Not for breeding, pregnant or lactating dogs or dogs less than 12 months of age.
Dogs should be monitored for the development of infections, including demodicosis and neoplasia. It is not a corticosteroid or an antihistamine. Its use has not been evaluated with the use of glucocorticoids, cyclosporine or other systemic immunosuppressive agents.


Apoquel trials by the company found the drug effective.
Effectiveness for the Control of Atopic Dermatitis
1. Treatment success for pruritus up to Day 28
2. Treatment for success for skin lesions up to Day 28.


Effectiveness for the Control of Pruritus with Allergic Dermatitis

History of allergic dermatitis is attributed to one or more of the following conditions:
atopic dermatitis, flea allergy, food allergy, contact allergy and other/unspecified allergic dermatitis.
