Running with the howling wolves

Do not let the doubts of fools and bootlickers
Stop you from lifting the universe

4101. Syrian Hamster - Traumatic testicular swelling

Oral meloxicam is effective as a pain killer as the Syrian hamster stopped licking his
testicular swelling. By Day 2 as an inpatient, the hamster had little swelling and was sent home.

4109. REYNA. Closed pyometra in a Syrian Hamster - A rare surgical condition

INTRODUCTION -  Singapore is a city state with around 6 million residents. Around 80% of the population live in high-rise apartments. Many prefer to keep hamsters as pets. Dwarf and Syrian hamsters are most popular.

Use SOME VIDEO footage showing Syrian AND dwarf hamsters IN A PET SHOP.

As the hamster ages, tumours are the No. 1 most common medical condition seen at Toa Payoh Vets over the past 10 years.

This video documents a rare case of closed pyometra in a Syrian hamster seen at Toa Payoh Vets.

"Hi hi Dr Sing, are you in today?

My female Syrian hamster has not been pooping and eating well for the past week and yesterday, I felt her tummy was quite swollen.

Can I bring her in around 11.30 am?"
-----------------
"11.30 am OK.
------------------
"Ok, I bring her over. Thanks"

At Toa Payoh Vets
"My Syrian hamster's tummy is getting larger and larger," the young lady told me that her female 1.5-year-old hamster was losing appetite and was lethargic over the past 2 weeks. "Is she constipated?"

I palpated the swollen abdomen with my fingers.
"There is bad news," I replied. "I felt a large sausage-like mass as big as the last finger of an adult lady. "Your hamster is likely to be suffering from closed pyometra."
"What is closed pyometra?"

"Pyometra is a bacterial infection of the uterus. Pus is formed. The pus usually passes out and you can see a yellowish vaginal discharge. Did you see any?"

"No, I did not see any discharge in her private parts," the owner said.

"In your Syrian hamster, the pus builds up daily, but cannot be passed out as the cervix has closed," I explained. "I need to get her abdomen X-rayed to confirm the closed pyometra. Is it Ok with you?"

X-rays showed a severely distended uterus inside the abdomen.

The left uterine horn (XY) is over-distended with pus. This X-ray shows incorrectly
that the left and right uterine horns are filled with pus, but during surgery, the right uterine
horn is normal, about 4 mm in diameter while the left horn is 1.5 cm in diameter and around 8 cm long! .

Hi, My hamster Elsa doesn't seem to get better.
Can she come on Friday (Mar 27, 2020) to take more medication or is surgery better?
The owner decided to feed the hamster more antibiotics.

VIDEO FOOTAGE OF PE-OP CONSULTATION

"There is only one treatment for closed pyometra in hamsters or dogs," I said to the young lady. "Surgery to remove the distended and infected uterus as drugs will not work to expel the pus."

"The other vet says that all hamsters will die when they undergo surgery," the owner had consulted another vet earlier. "I do not want surgery."

"Your hamster has been lethargic and not eating for at least one week. In this case, she is unfit to undergo anaesthesia as the risk of dying on the operating table will be very high."

SURGERY

After around 10 days of antibiotics and syringe feeding, the hamster's abdomen got even bigger. "My hamster's tummy is getting bigger and she is heavier," the owner consulted me at Toa Payoh Vets.

"This is surprising. At home, she sleeps all the time, but at the clinic now, she is active!"

"You have two options," I said. "Surgery to remove the swelling uterus which you saw on the X-rays. Or you can wait till the uterus over-distend till it ruptures or euthanasia her when she is very ill."

"It will be very painful for her if her uterus ruptures," the young lady loved this Syrian hamster very much. "You know what? I understand the high risk of anaesthesia. I opt for the spay surgery."

"Even if the hamster survives the surgery, she may die after surgery as she is in poor health," I advised her. "She is not in good health as she does not eat on her own."

"I have to take the chance. I will take the surgery," the owner decided

She signed the consent form.. The hamster was given 0.01 ml of Zoletil 100 IM and put on isoflurane gas anaesthesia by mask. The left large uterine horn filled with pus. I took it and its ovary out. The other uterine horn was normal and had no pus. I removed it and its ovary.

The left uterine horn is filled with pus and is around 1.5 cm in diameter
and 8 cm long while the right horn is normal at 4 mm in diameter.

Just after spay

The Syrian hamster was active post-spay and went home with instructions to keep her warm using a plastic bag with warm water to place near her body. If she could survive the next 2 days, she should live.

VIDEO FOOTAGE OF POST-OP AND OTHER VIDEOS TO BE EDITED

Unfortunately, she passed away the next day.

TIPS AND ADVICES
In closed pyometra, especially in dogs, the earlier the surgery to remove the toxic uterus, the better the chances of survival. The dog has better chances if she is eating some food and is not vomiting.
(TWO IMAGES OF DOG CLOSED PYOMETRA)

In this rare case of closed pyometra in the Syrian hamster, the hamster survived the anaesthesia and surgery which was a very difficult decision for the owner to make.

Dear Dr Daniel Sing

Yesterday, we brought KAME for consultation and injection to induce her to lay eggs.

Please help to review the X Ray again for KAME if she had 8 or 9 eggs?  So far she laid 8 (laid first egg about 5 o clock [about 1 hr 15 mins after injection] and continuously next 45 minutes) in the car at Lorong 5 car park. It was raining heavily.

We brought her home and she fell asleep without food.

This morning Rita found her in the bathtub but the water drained.  After filling up the bathtub with water, she was restless and not eating yet.

Thank you.

Best regards
Kho / Rita

Dear Doctor Daniel Sing

Finally the biggest egg, KAME laid around 4pm this afternoon. She dug a hole at the garden and laid and covered it.  Rita took it out from the mud and took a photo of it for your record.

Thank you.

Best regards
Jimmy / Rita

PREVIOUS OXYTOCIN INJECTION IN DEC 2019

APRIL 20, 2019

Dear Dr Sing
We went back about 5pm and since you administered the injection she had laid 5 eggs. As we were travelling home she laid one egg in the car.
When my wife reached home about 5.45pm she laid another 2 eggs. We went out and when my son reached home, he found 4 eggs in the plastic box (filled with water) around 6.45pm.
In total she laid 12 eggs. Is it possible for her to form another 7 eggs in 11 days?
She is very tired and now sleeping.   We are monitoring her closely if more eggs 🥚 to come.

4106. HABITAT LOSS. The Oriental Garden Lizard loses his home in Yio Chu Kang Crescent

 Photography Tips.

It is too much to carry a camera bag with many lens when taking local wildlife photography.

I use one one zooms lens from 18mm to 250 mm, DSLR Canon camera. The tip is to use the setting to "P" Mode. This will be easier for amateurs to take good photographs of wildlife without having to figure out the shutter speed or aperture.

The follow images are taken with this zoom lens.

Mar 6, 2020

The regular and once-monthly clearing of the vines, creepers, grass and leaves from the railing and the nearby areas of Lentor Avenue/Yio Chu Kang Crescent result in the loss of habitat for this young Oriental Garden Lizard. He has no insects to hunt and survive. There are very few of such creatures nowadays, less than 5 in this area which is being developed into a major road. The lizard will become extinct as Singapore develops more land and build more.

Mar 20, 2020. Rare sighting of the oldest Oriental Garden Lizard seen by luck, being at the right place at the right time. How do I know? Its length is longer and its head is broader compared to the younger ones. (show photo side by side).

-------------------------------------------

On the same day, at the Countryside Park Garden, there was a younger adult. Hunting.

----------------------------------------------------------

As the habitat is lost owing to construction of the highway, these lizards are getting extinct.

Jun 4, 2020.
A good conditioned Oriental Garden Lizard is seen along Yio Chu Kang Crescent, opposite the Seletar Simpang River. He seemed to be shedding his old coat.

This Oriental Garden Lizard was not bothered with me taking videos and photographs. Once, he decided to hop down from the ropes. After that he went up the ropes and onto the barrier pole!

The following lizard could be the same one as the above on the barrier pole but on different dates.

Surgery is the only option. Seek a hamster vet who performs surgery.
Buy cream from pet shops or drugs from the vet will not make the warts disappear.

An auroscope may be used to check the inside of the mouth of the guinea.
Special dental instruments are used to open the mouth of the guinea pig wide for inspection.
Dental X-rays are ideal. If not available, do the skull X-ray.
Antibiotics, drainage, and surgery are all common treatment for jaw abscesses.

4102. REYNA. Burring of the overgrown front teeth in a Guinea Pig

1.  Edit the video - show the use of the burring tool to trim the excess length. Burring is said to reduce the likelihood of tooth fracture if one uses just a cutter.

Overgrown front teeth

burring of the overgrown front teeth under anaesthesia

A guinea pig grows continuously unlike human teeth. They are worn down by the guinea pig eating hard food like hay. However, this guinea pig was not given sufficient hay and the teeth grew and grew. They overlap and the guinea pig cannot eat properly. He loses weight. This vet educational video shows the burring or cutting of the overgrown teeth to enable the guinea pig to eat normally again. After the procedure of burring using the Dremel 300 cutting wheel as shown in this video, the guinea pig now can chew hay normally and has gained weight. Early signs like loss of appetite warn the owner that the teeth are overgrown. She sought Toa Payoh Vets for early treatment and now the guinea pig is thriving.
